Center for Community Health Equity Accelerator Fellowship 2026/27

Applications are open for the M Health Fairview Health Equity Accelerator Fellowship. This 12-month program funds community leaders addressing health equity locally. Eight fellows join a cohort building real, tested community projects. Projects must be implemented within Fairview’s Minnesota service area.

About the Fellowship

This fellowship backs Minnesota leaders tackling health equity in their own communities. Fellows don’t need a fully developed project to apply successfully. Coaches and experts help fellows build and refine ideas throughout the year. Past fellows have led work in nutrition, housing, and recovery support.

What Fellows Receive

Fellows receive up to $10,000 to fund their community project. Structured curriculum covers health equity and program development directly. Coaching and technical assistance come from experienced field experts. Fellows also build lasting networks with peers and aligned professionals.

Who Can Apply

Applicants need a health equity project idea for Fairview’s service area. Candidates must be at least 18 and not employed by Fairview. Relevant experience or community ties to the issue are required. Full attendance at all fellowship sessions throughout 2027 is mandatory.

Required Materials

Applicants submit basic information, two recommendation letters, and narrative responses. A draft project budget using Fairview’s template is also required. Reviewing the full application form beforehand is strongly recommended.
